Coaching Options

Option #1 Introductory Special: One 30-minute telephone consultation and 2 e-mail communications. Get to know your Wellness Coach and enjoy her undivided attention to your specific health concerns and questions. Fee: $45.00
Option #2: “Personal Health Blueprint” plus a total of 10 e-mail communications within the following 8 weeks. Fee: $150.00

Option #3: “Personal Health Blueprint” plus three 30-minute telephone consultations and unlimited e-mail communications during the initial month. Fee $275.00

Option #4: After completion of Option #2 or Option #3, you may choose ongoing unlimited e-mail communications plus three 30-minute telephone consultations per month.
Fee: $135.00 per month

Option #5: After completion of Option #2 or Option #3, you may choose 10 e-mail communications per month. Fee: $45.00 per month

All fees are paid in advance.

  • Print, fill out, and fax or mail the Wellness Coaching Agreement Form.
  • Print, fill out, and fax or mail the Credit/Debit Card Pre-Authorization Form.
  • For Options 2 and 3, please print, fill out in detail, and fax or mail the three questionnaires on the tabs to the left, along with your two forms. This information will provide the foundation for developing your “Personal Health Blueprint”. The more details you provide, the more effective your Blueprint will be.
  • After receiving your three questionnaires and two forms , Teresa will create for you a comprehensive “Personal Health Blueprint” for you which will provide the tools you need to achieve your health improvement objectives.  It will also include a variety of resources and handouts to help you develop a healthy lifestyle to utilize for life!
